Clothing Addons

So a server I play on is attempting to allow clothing addons for multiplayer skins in order to create uniforms for different factions. Does anyone know how to do this?
Just to clarify, we aren't looking to replace stock models/textures, we're looking to create our own and add it to the FiveM server. Texturing/Modelling isn't a problem, we're just confused as to how to make a custom multiplayer t-shirt for instance, and have that t-shirt as an addon as opposed to a replacement.
