Creating custom clothing textures.

Ok, so I have tried and tried to take a vanilla clothing model and add in a custom graphic. When I try to add it in the files via OpenIV it just corrupts my game. I have tried to replace the original file with a png, corrupted, then I tried dds, corrupted. What the hell am I doing wrong? I've had no issue modding a base game before, but I have never tried doing my own clothing. HELP!
