If the answer to JohnFromGWN's question is the former, all you need to do is create a list of fat ped models then suppress them all using SET_PED_MODEL_IS_SUPPRESSED/

@Jitnaught i vote for your suggestion. It's more humane than the Obese Ped Genocide mod, or OPG. There is also a Fatty Liposuction Mod but it's outdated and dangerous. Of course if the OP isn't impatient the Los Santos Low Carb limit adjuster is pretty effective for weight loss and works smoothly with the other weight management mods, i.e. heap and pack.
