Script Mod Help needed

Hello everyone
I am looking similar in GTA V .Net coding

eg. from GTA 4
Inherits GTA.Forms.Form

I want show some label, radio, checkbox, input, button, color dialog

Radio? I don't think I've seen this in any GTA5 mod but should be possible since all the other controls are possible. Checkbox is commonly used and essentially will be a workaround for radio.
