Yeah, that don't sound good. Think your game is bipolar.
Here' s a few ideas:
When you reinstalled the game previously have you installed it to the same location as before? I mention this as I have experienced certain GTA V issues persisting through a reinstall & only a change of install location was able to get rid of them.
Try a completely different gameconfig from a different author. Nothing wrong with the one you linked to but you're looking for anything that can change an outcome. Maybe your game just don't like the way that one is edited.
Might be worth running sfc /scannow & chkdsk just to make sure Windows & your game install hard drive are in good shape. Run chkdsk on all your hard drives, not just the install one/s.
Remove any overclock you might have on any components before reinstalling the game again (do the same thing if reinstalling Windows also).
HCI Memtest is also good to show if your memory is working correctly. It can be run while logged in (no need to restart/boot from CD etc). Put '2048' in 'Enter megabytes of RAM to test' box & then run multiple instances to test more RAM at the same time to cover all your RAM quicker. However you do it, it'll test all your RAM eventually.
Download SUPERAntiSpyware Free Edition google street view (Free Edition download link on the right, ignore the 'free trial' link) & run a full scan of your computer. For me, this has found things that Kaspersky & other software have missed.
Try using msconfig to perform a clean boot of Windows & then try reinstalling some add-ons & see if you get the same results?
Covered already, but always use newly downloaded files. Don't use anything from the files you have saved previously. Not a single thing.
Try installing a few add-ons to the game folder rather than the 'mods' folder & see if you get a different outcome.
Reseat (remove & reinstall) your GPU & RAM.
As a last ditch effort reinstalling a completely fresh copy of Windows might be worth a try (an in-place install will carry over registry settings etc from previous install & should be avoided in this case. Do a full format 100% fresh new install using a freshly downloaded version of the Windows install media).
